Contact our Customer Service Department at (800) 207-8050 or log on to the UNL Customer Portal to download your claim form. You will need your policy number to complete your claim form.
A family member, beneficiary or funeral home should call our Customer Service Department at (800) 207-8050 with the following information:
- Insured’s name
- Policy number
- Date of death
Customer Service will verify we have a life policy in force for this individual and will advise the necessary documentation needed to file the claim.
Initial documentation needed includes:
- Completed claim form, including HIPAA form, but the beneficiary or beneficiaries
- Original, certified death certificate
- Copies of any police or autopsy reports (if applicable)
Our Customer Service Department will send a letter listing the documents needed and provide the necessary claim form. Once this documentation is returned to UNL, your Claim Support Representative will review the claim and notify you if any additional forms or information are required.
If your health policy has been lapsed for more than six months, policy reinstatement is unavailable. If your policy has been lapsed for less than six months, you may apply for reinstatement.
Please call our Customer Service Department at (800) 207-8050 and we will provide you with the necessary reinstatement form that you will need to complete as well as the premium amount needed for reinstatement. When the completed reinstatement application and premium have been received, our Underwriting Department will review. If approved, the premium will be applied and your policy will be reinstated; if it is not approved, your premium will be returned to you.
Yes, call our Customer Service Department at (800) 207-8050 to verify your address and we will send you a new one. There will be a $25 fee for your replacement policy.
